This ghost picture looks like a smoky skeleton ghost reaching toward the girl in the photograph.

Cigarette smoke can often be captured in photos and confused with being ghostly, ectoplasm mists.  Given the likelihood that the bar had people smoking in it, we would guess this smoky ghost image was caused by cigarette smoke residue in the atmosphere.  However, this ghost picture is intriguing and one which we just love, so you decide for yourself what caused the smoky figure...
